Give an AI assistant access to the context and it still acts first and looks second. Retrieval that is optional is retrieval that gets skipped under time pressure — by people too. Memory has to be handed over at the start, not offered on request.

A language model asked about something it was never told does not stop. It answers from its training data, fluently and with confidence. We watched one call this company by a name it had abandoned months earlier — and it had no way to know.

Written down is not the same as current

A rule gets written into a document. Later the rule changes, and the document does not. Nothing breaks, nothing warns — the two just quietly say different things until somebody acts on the wrong one. The older the document, the more it is trusted.

Every conversation with an assistant starts from nothing, so somebody pastes in the background again, explains the same constraint again, and catches the same mistake again. The cost is not the mistake. It is the most experienced person in the room spending their week as a human clipboard.

It says what it does not know

A briefing names its own gaps: what was left out, what the sources disagree on, how fresh each part is. An answer that hides its holes is worse than no answer — you cannot tell when to go and check.

Retrieval is only half of it. At the end of the work the assistant writes back what was decided, what turned out to be a constraint, what was left unfinished — to the project, not to the conversation. That is the half that makes next week cheaper than this one.

What an assistant may retrieve is bounded by what the person asking is allowed to see. Visibility is part of the picture, not a filter bolted on top.

The core service makes no calls to any language model and sends no telemetry. It is a Go binary and a database — verifiable from the dependency file.

Software team. An AI assistant picks up a task. Without the picture it reads the ticket and the code, and never learns that the same question was settled differently three weeks ago in a document, or that the decision was later overturned. It writes entirely plausible work on a wrong assumption.

With it, the work starts from what holds: these decisions apply, this one is the most recent, and here two sources disagree — the document says one thing, the decision another, and the decision is preferred because it is newer and was made by the person entitled to make it.

Consultancy, agency, services. The promise is in a proposal. The work is somewhere else. The decision that narrowed what was included is in a third place, made by someone who was not in the customer meeting. Joining those up is manual work, and it usually happens after something has already gone wrong.

Any organization. The usual answer is a status meeting, where everyone reports what they remember, the recollections differ, and some of what matters is known only to someone who is not in the room. Afterwards everyone shares a picture — for about a week.

With it, the briefing is composed across the sources before anyone meets: what is settled, what is open, where the sources contradict each other — and what is not written down anywhere, which is the part that has to be asked from a person.

Onboarding, any field. The induction material explains how. It does not explain why — the reasoning sits in people’s heads and in old conversations. A new person either interrupts somebody or guesses. When someone leaves, the reasoning leaves with them.

With it, the assistant gives the decisions that are in force with their reasoning — including what was considered and rejected, and why — filtered to what that person’s role is entitled to see. The same mechanism answers the other question everyone has: what is stored about me?

PanelPaya puts the same question to several models and shows where they agree and where they do not. Disagreement is the useful output — it flags where a human should look.
